Medvi

Is Medvi a zero-employee company? No. Medvi has at least one employee besides the founder, uses contractors, and relies on human-staffed partner networks for clinical care, pharmacy, fulfilment and compliance. It is a lean marketing and checkout storefront, not an AI-run zero-employee company.

Medvi (MEDVi, LLC) is a US direct-to-consumer telehealth brand that markets and sells cash-pay GLP-1 weight-loss treatments, including compounded semaglutide and tirzepatide. Clinicians in partner networks prescribe, and partner pharmacies compound and dispense the drugs.

How each function is executed

Rated only where a source describes the function. Unknown is not low.

  • EngineeringMedium

Gallagher reportedly used ChatGPT, Claude and Grok to write the platform's code and built AI agents to connect systems. Reports also say he uses contract engineers, and that he fixed a broken checkout himself.[7],[8]

  • ProductUnknown

No source describes how this function is executed.

  • MarketingMedium

Ad images and video were generated with Midjourney and Runway, and website copy with AI. Customer acquisition also ran through affiliate marketers paid per lead or acquisition, such as The Offer under a September 2024 insertion order. Medvi blamed affiliates for marketing problems.[7],[8],[9],[3]

  • SalesUnknown

No source describes how this function is executed.

  • SupportMedium

Reports say customer service runs on ElevenLabs and custom AI agents. The chatbot reportedly 'made up drug prices and hallucinated products'. Contract account managers are also reported, and the website says care coaching is 'led by certified medical assistants and registered dietitians'.[8],[7],[12]

  • FinanceHuman

Gallagher initially used AI accounting tools and later replaced them with an accounting firm.[7]

  • LegalHuman

Gallagher initially used LegalZoom and later replaced it with a law firm. Medvi is party to several lawsuits.[7],[9]

  • OperationsHuman

Clinical and fulfilment work is outsourced to human-staffed partners. Medvi's site says 'Medical treatment is provided by CareGLP Affiliated P.C.s, and OpenLoop Health'. It names partner pharmacies Triad Rx, RedRock Pharmacy and Beaker Pharmacy & Compounding, and says 'MEDVi is not acting as a pharmacy'. The FDA letter confirms Medvi is not the compounder. Reports say CareValidate and OpenLoop handle doctors, prescriptions, pharmacies, shipping and compliance.[12],[1],[2],[4]

Operating model

How the company operates

Medvi owns the brand, website, checkout, customer relationship and marketing. Licensed physicians, prescription processing, pharmacy compounding and dispensing, shipping and regulatory compliance are provided by partner networks: OpenLoop Health, the CareGLP affiliated professional corporations, and CareValidate. Named compounding pharmacies include Triad Rx (AL), RedRock Pharmacy (UT) and Beaker Pharmacy & Compounding (TX). Onhealthcare.tech says Medvi 'rents all of that by the transaction'.[12],[4],[5],[2]

Acquisition relied partly on affiliate marketers paid per lead or acquisition. An affiliate firm's court filings say the arrangement drove tens of millions of dollars in revenue to Medvi per month. Medvi is in a contract dispute with that firm.[9]

Regulatory and legal exposure: an FDA warning letter (2026-02-20) for false or misleading claims that implied Medvi compounds the drugs and that compounded products are equivalent to approved ones. Medvi faces a California anti-spam class action (2026-03), TCPA suits and a contract suit. In May 2025, Futurism reported that Medvi claimed a partnership with a doctor who 'had no involvement with the company', and that it manipulated photos into before-and-after images.[1],[3],[9],[11]

Founder responsibilities

  • Built the website and software using AI coding tools, produced ad creative with AI, and set up AI customer-service tools and internal performance dashboards.[7],[4]
  • Hired his younger brother Elliot as the company's only employee.[7],[2]

16 Oct 2026Customers
period 2026-10
“Join 500,000+ MEDVi patients”

Marketing copy on the company homepage, read on the access date. The page also notes that testimonial images 'may use models'.

MEDVi [12]
5 Apr 2026Funding raised
period 2026-04
“has raised no outside funding”

Started with about $20,000 of the founder's money. Telecare Aware says 'Medvi has not borrowed and has no investors, thus no external valuation.' No valuation exists; the '$1.8 billion' figure refers to projected sales.

3 Apr 2026Contractors
period 2026-04
“uses contract engineers and account managers”

Number not disclosed. Il Post says he now also uses contract collaborators. The Neuron (src-medvi-7) says AI accounting tools and LegalZoom were later replaced by an accounting firm and a law firm. Separately from contractors, clinical, pharmacy and shipping work is done by partner organisations (see autonomy and operations).

Il Post [11]
3 Apr 2026Annual revenue“has reached $180 million in revenue with only two full-time employees”

Crypto-news aggregator summary (and KuCoin translations) that gives no period and names no author. It is almost certainly a garbling of the $1.8B projection. Recorded for completeness only; do not use.

2 Apr 2026Founders
period 2024-09
“Matthew Gallagher started Medvi from his Los Angeles home”

From the NYT profile of 2026-04-02 (Erin Griffith), as relayed by Moneywise and Il Post. The NYT page could not be fetched; the domain is blocked for our tools.

Il Post [11]
2 Apr 2026Employees
period 2026-04
“Medvi only has two employees (Matthew and his brother Elliot)”

Sources say 'two employees', counting the founder; the brother is the only hire. Employee_count excludes the founder, so it is 1. This originates with the company/NYT and was repeated by several outlets, with no independent headcount check. Dealroom phrases it as one employee besides the founder.

2 Apr 2026Team size
period 2026-04
“two employees — Gallagher and his brother Elliot”

Excludes contractors, contracted accounting and law firms, affiliate marketers, and the staff of the partner clinical and pharmacy networks.

2 Apr 2026Annual revenue
period 2025
“$401 million in sales in 2025”

Company-reported to the NYT; Telecare Aware says the company 'reported to the NYT'. Implicator.ai says 'The Times verified Medvi's 2025 financials', and onhealthcare.tech says the NYT verified them. I could not open the NYT article to confirm its own wording about reviewing documents. If confirmed, this would qualify for grade B. Partial consistency: affiliate firm The Offer's court filings (per src-medvi-9) say its traffic drove 'tens of millions of dollars in revenues to Medvi per month'.

2 Apr 2026Profitability
period 2025
“$65 million in net profit, 16.2% margins (2025)”

Value is 2025 net profit in USD as reported via the NYT. Same verification caveat as obs-medvi-annual_revenue-1.

2 Apr 2026Revenue projection
period 2026
“on track to do $1.8 billion in sales this year”

This is a founder projection or run rate, not a closed result: 'Matthew Gallagher's own 2026 projection, not audited revenue' (implicator.ai). It is not a valuation, though headlines called Medvi a '$1.8 billion company'. Dealroom and onhealthcare cite 'more than $3 million per day' in early 2026, without a primary source. Reclassified from run rate: a forward-looking projection for 2026 sales, not an observed run rate.

2 Apr 2026Customers
period 2025-12
“250,000 customers by end of 2025”

Company-reported via the NYT.

Open questions

What we could not verify

  • Did the NYT itself state that it reviewed Medvi's financial documents? Secondary sources say it 'verified' the 2025 numbers, but the NYT text could not be accessed. Confirmation would upgrade revenue and profit to grade B.
  • Is the $401M gross consumer sales, including the partner-network share for physician services and drugs, or Medvi's net revenue after paying OpenLoop, CareGLP and the pharmacies?
  • Number of contractors (engineers, account managers) and whether any staff are employed through partners or a PEO.
  • Customer concentration and economics with OpenLoop and CareValidate, and Medvi's exposure to OpenLoop's lawsuits and data breach.
  • Outcome of the FDA warning letter (no public response or close-out found) and the effect of the FDA's 2026 crackdown on compounded GLP-1 marketing on 2026 sales.
  • Actual 2026 revenue versus the $1.8B projection.
